I/We understand that in order to apply for a joint debit card, the checking account must also be a joint account. I/We also certify that the above information is true and agree to abide by the terms of the debit card agreement and disclosure statement which is atttached to this applicant and should be kept for my records. I/We also authorize C&O UnitedCredit Union to obtain credit reports. The Credit Union will provide the name and address of any credit bureau used to obtain credit reports upon request.

MEMBER AGREEMENT

     I(we) request the C&O United Credit Union ("Credit Union") to issue the MasterMoney Card ("Card") subject to the following terms and conditions.
     The Card and Machines are provided solely as a convenience to Member(s) for making deposits, payments, transfers of funds, and withdrawals, involving such checking, savings, and/or loan acoounts as are encoded on the Card when issued. All terms and conditions of the written agreements between Credit Union and Member regarding those accounts apply to all transactions in which the Card is authorized for use.
     The location and allowable usage of ATM machines shall be determined and/or changed by the Credit Union at any time without prior notice to the Member The Card and security code remains the property of the Credit Union and the Credit Union may terminate the Member's right to use the Card and/or security code at any time. Upon the demand of the Credit Union the Member will return the Card and security code to the Credit Union. The Card may be superseded by any renewal or replacement cards issued for that purpose by the Credit Union but all rights and privileges extended under this agreement shall terminate on the expiration date to be determined by the Credit Union or the termination of agreement between MPS of Cincinnati, Ohio, and the Credit Union.
     MACHINE PROCEDURES: The Machine shall be operated as set forth by the instructions furnished to the Member. The Machine will only be used to effect transactions the Credit Union programs the Machine to accept and subject to limitations of dollar amounts and frequency use as the Credit Union may from time to time determine. The accounts to be associated with the Machine will be those agreed to by the Member with the Credit Union as specified on the reverse of this agreement. Use of the Machine shall be evidenced by a record of each transaction. All Machine transactions will be verified by the Credit Union and entered on actual Credit Union account records. Deposit receipts are issued by Machine subject to verificafion and amendment by Credit Union to effect daily reconcilation of cash actually received. All transactions authorized under this agreement are considered as ordinary Credit Union procedure.
     MEMBER RELATIONS: Credit Union's a liabiy Is limited to its undertakings in the separate written agreements governing use of the loan and/or share accounts whose account identification numbers are encoded upon the Card at the time of issue and no additional liability is created or warranty given,either expressed or implied through issuance of the Card, these terms and conditions or any regulations by Credit Union regarding its use or use of the Machine.
     In no event shall member write Member's password on the card or keep any written record of it on any material which is kept with the card. If the Member informs the Credit Union within 24 hours after learning of the loss or theft of the Card, the Member will lose no money if the Card is used without the Member's permission. If the Member does NOT tell the Credit Union within 24 hours, then the Member could lose as much as $50. Also if the Member's statement shows transfers that the Member did not make, tell the Credit Union at once. If the Member does not tell the Credit Union within sixty (60) days (after the statement was mailed), the Member may not get back any money lost after the sixty (60) days if the Credit Union can prove that it Could have stopped someone from taking the money if the lost or stolen Card had been reported in time. If a good reason (such as a long trip or a hospital stay) kept the Member from reporting to the Credit Union, then the time period will be extended.
ERROR RESOLUTION
     In case of errors or questions about your electronic transfers, call or write us at the telephone number of address listed on this web page as soon as you can, if you think your statement or receipt is wrong or if you need more information about a transfer listed on the statement or receipt. We must hear from you no later than 60 days after we sent the FIRST statement on which the problem or error appeared.
          1) Tell us your name and account number.
          2) Describe the error or the transfer you are unsure about, and explain as clearly as you can why you
          believe it is an error or why you need more information.
          3) Tell us the dollar amount of the suspected error.

     If you tell us orally, we may require that you send us your complaint or question in writing within 10 business days.
     We will tell you the results of our investigation within 10 business days (20 business days if the transfer involved a point-of-sale transaction or a foreign initiated transfer) after we hear from you and will correct any error promptly. If we need more time, however, we may take up to 45 days (90 days if the transfer involved a point-of-sale transaction or a foreign initiated transfer) to investigate your complaint or question. If we decide to do this we will recredit your account within 10 business days (20 business days if the transfer involved a point-of-sale or a foreign initiated transfer) for the amount you think is in error, so that you will have the use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or questions in writing and we do not receive it within 10 business days, we may not recredit your account.
     If we decide that there was no error, we will send you a written explanation within three business days after we finish our investigation. You may ask for copies of the documents that we used in our investigation.
     If Card is lost or stolen Member shall notify Credit Union in writing by fax or letter to C&O United Credit Union, 3029 Dixie Highway, Edgewood, KY 41017, Attn: Share Draft Department, Telephone (606) 331-3447 or 800-964-8205; Fax (606) 578-3642, 9:00a.m. to 5:00p.m., Monday - Wednesday; and 9:00 a.m. - 6:00 p.m. Thursday - Friday, except holidays; 1-800-528-2273 evenings, weekends.
     AGREEMENT. I agree to the release of personal information to the extent where it is necessary for completing transfers; or in order to verify the existence and condition of my account for a third party, such as a credit bureau or merchant; or in order to comply with government agency or court order; or if I give the Credit Union written permission.
     Credit Union reserves the right to arnend and modify this Agreement. My amendment or modification shall become effective on the date specified therein. Member's retention of the Card after such notice shall constitute agreement to all matters included within the notice.
     Member may surrender the Card (only by surrendering all Cards issued on the account) and terminate this Agreement. However this Agreement shall remain in effect for purchases made and services rendered by or to any User using the Card.
     Use of this Card after notice of its revocation is fraudulent and will subject Member or any User to legal proceedings.
